IV infusion Adult Herpes simplex (except herpes encephalitis), varicella zoster infection 5 mg/kg every 8 hr.
Herpes encephalitis, immunocompromised patient w/ varicella zoster infection 10 mg/kg every 8 hr.
Childn Herpes simplex infection 250 mg/m
2 every 8 hr for 7 days.
Immunocompromised childn w/ varicella zoster infection 500 mg/m
2 infused over 1 hr 8 hrly for 7 days.
Childn 6 mth-12 yr w/ herpes simplex encephalitis 500 mg/m
2 infused over 1 hr 8 hrly for 10 days.
HSV infection Renal impairment: CrCl >50 mL/min 5 or 10 mg/kg every 8 hr;
25-50 mL/min 5 or 10 mg/kg every 12 hr;
10-25 mL/min 5 or 10 mg/kg every 24 hr;
0 (anuric)-10 mL/min patient receiving continuous ambulatory peritoneal dialysis (CAPD) 5 or 10 mg/kg should be halved & administered every 24 hr,
patients receiving haemodialysis 5 or 10 mg/kg should be halved & administered every 24 hr & after dialysis.
