Indications/Uses
Herpes simplex, primary & recurrent varicella zoster infections in immune-compromised patients. Prophylaxis of herpes simplex infections in severely immune-compromised patients. Shingles (recurrent varicella zoster infection) in patients w/ normal immune response. Herpes simplex encephalitis in patients >6 mth.
Dosage/Direction for Use
IV infusion Adult Herpes simplex (except herpes encephalitis), varicella zoster infection 5 mg/kg every 8 hr. Herpes encephalitis, immunocompromised patient w/ varicella zoster infection 10 mg/kg every 8 hr. Childn Herpes simplex infection 250 mg/m2 every 8 hr for 7 days. Immunocompromised childn w/ varicella zoster infection 500 mg/m2 infused over 1 hr 8 hrly for 7 days. Childn 6 mth-12 yr w/ herpes simplex encephalitis 500 mg/m2 infused over 1 hr 8 hrly for 10 days. HSV infection Renal impairment: CrCl >50 mL/min 5 or 10 mg/kg every 8 hr; 25-50 mL/min 5 or 10 mg/kg every 12 hr; 10-25 mL/min 5 or 10 mg/kg every 24 hr; 0 (anuric)-10 mL/min patient receiving continuous ambulatory peritoneal dialysis (CAPD) 5 or 10 mg/kg should be halved & administered every 24 hr, patients receiving haemodialysis 5 or 10 mg/kg should be halved & administered every 24 hr & after dialysis.
Contraindications
Hypersensitivity to aciclovir or valaciclovir.
Special Precautions
Not to be administered orally. Dehydrated or renal impairment. Pregnancy & lactation. Elderly.
Adverse Reactions
Phlebitis; nausea, vomiting; reversible increased liver-related enzymes; pruritus, urticaria, rashes (eg, photosensitivity); rapid increases in blood urea & creatinine levels.
Drug Interactions
Increased plasma conc w/ drugs competing the elimination via active renal tubular secretion. Increased AUC & reduced renal clearance by probenecid & cimetidine. Concurrent administration w/ drugs that compete aciclovir elimination. Increased plasma AUC of mycophenolate mofetil. Concomitant use w/ drugs affecting other aspects of renal physiology (eg, cyclosporin & tacrolimus).
